Hillsdale Teacher/Staff Profile Questionnaire

Thank you for your interest in participating in our Teacher/Staff Profile Series!

Background: The Hillsdale Teacher and Staff Profile Series is a new program through which the PTO will publish mini-interviews with Hillsdale teachers and staff who volunteer to participate. The goal of the program is to highlight the amazing and talented teachers and staff who do so much to inspire and support our Hillsdale students. Each mini-interview will be published on our website and Facebook page. We hope to have enough interest to publish a new profile each week. Please consider participating!

Instructions:

1. Fill out the form below with as much information as you wish to provide.  We do ask that you answer at least 4-6 of the questions. 
2. Choose a photo to be published with your profile. If you do not wish to include a photo of yourself, consider a photo of a pet, a photo of a favorite vacation spot, something related to a hobby, etc.
3. E-mail a high-resolution version of the photo of your choosing to Nancy Wood at [email protected]. 
4. We will send you a draft of the profile for review and feedback before it is published.
